UMass Medical School iHeal project uses emerging technologies to detect drug cravings and intervene

WORCESTER, Mass. Clinical researchers at the University of Massachusetts Medical School (UMMS) are combining an innovative constellation of technologies such as artificial intelligence, smartphone programming, biosensors and wireless connectivity to develop a device designed to detect physiological stressors associated with drug cravings and respond with user-tailored behavioral interventions that prevent substance use. Preliminary data about the multi-media device, called iHeal, was published online first in the Journal of Medical Toxicology.

According to the study's authors, many behavioral interventions used to treat patients are ineffective outside of the controlled clinical settings where they are taught. This failure can be attributed to several factors, including a patient's inability to recognize biological changes that indicate increased risk of relapse and an inability to change their behaviors to reduce health risk.

Edward Boyer, MD, PhD, professor of emergency medicine at UMass Medical School and lead author of the study, worked with colleagues at UMMS and at the Massachusetts Institute of Technology, to design a mobile device using so-called "enabling technologies" that could be used to make behavioral interventions for substance abusers more effective outside the clinic or office environments. The result of their work, iHeal, combines sensors to measure physiological changes and detect trigger points for risky health behaviors, such as substance use, with smartphone software tailored to respond with patient-specific interventions.

Individuals with a history of substance abuse and post-traumatic stress disorder were asked to wear an iHeal sensor band around their wrist that measures the electrical activity of the skin, body motion, skin temperature and heart rate all indicators of stress. The band wirelessly transmits information to a smartphone, where software applications monitor and process the user's physiological data. When the software detects an increased stress level, it asks the user to annotate events by inputting information about their perceived level of stress, drug cravings, and current activities. This information is then used to identify, in real-time, drug cravings and deliver personalized, multimedia drug prevention interventions precisely at the moment of greatest physiological need.

Boyer and his teams examined the iHeal system architecture, as well as preliminary feedback from initial users, to identify key attributes and assess the device's viability. (AP) ? Greece won a second massive financial bailout in the early hours of Tuesday morning when its partners in the 17-country eurozone finally stitched together a euro130 billion ($170 billion) rescue, meant to avoid a potentially disastrous default and secure the euro currency.

But the patchwork of measures, including the implementation of more austerity in Greece and approval by skeptical German and Dutch Parliaments, required to give the rescue even a chance of success mean that it's unlikely to be the end of the continent's debt crisis.

Markets responded cautiously to the long-awaited deal, though the euro managed to eke out some gains as the fear of a disorderly Greek debt default within a month diminished.

The finance ministers from Greece and the other 16 countries that use the euro as their currency wrangled until the early morning hours over the details of the rescue, squeezing last-minute concessions out of private holders of Greek debt.

The eurozone and the International Monetary Fund, which will be providing the money for the new bailout, hope the new program will eventually put Greece back into a position where it can survive without external support and secure its place in the euro currency union.

The accord, which had been months in the making, seeks to reduce Greece's massive debts on all fronts, with both private and official creditors going beyond what they had said was possible in the past.

On top of the new rescue loans, Athens will also ask banks and other investment funds to forgive it some euro107 billion ($142 billion) in debt, while the European Central Bank and national central banks in the eurozone will forgo profits on their holdings.

But despite those unprecedented efforts, it was clear that Greece, which kicked off Europe's debt crisis two years ago, was at the very best starting on a long and painful road to recovery. At the worst, the new program would push the country even deeper into recession and see it default on its debts further down the line.

"It's not an easy (program), it's an ambitious one," said Christine Lagarde, the head of the IMF, adding that there were significant risks that Greece's economy could not grow as much as its international creditors were hoping.

The eurozone ? and Greece ? had been under pressure to reach an accord quickly to prevent Athens from defaulting on a euro14.5 billion ($19.2 billion) bond payment on March 20. The fear is that an uncontrolled bankruptcy even of relatively small Greece could unleash market panic across the rest of the continent. That would further unsettle other struggling countries like Ireland, Portugal or the much bigger Italy or Spain.

Despite the promise of new rescue loans, which come on top of a euro110 billion ($146 billion) bailout granted in 2010, the other 16 euro countries made clear that their trust in Greece is running low. Before Athens will see any new funds, it has to put into practice a whole range of previously promised cuts and reforms.

More significantly, Greece will have to pass within the next two months a new law that gives paying off the country's debts legal priority over funding government services. In the meantime, Athens has to set up a kind of escrow account, managed separately from its main budget, that will at all times have to contain enough money to service its debts for the coming three months.

These requirements, together with tighter on-the-ground monitoring, are an unprecedented intrusion into the fiscal affairs of a sovereign state in Europe and could eventually see Greece being forced to pay interest on its debt before compensating teachers, doctors and other state employees.

Greek politicians nevertheless greeted the package as a turning point for their battered country.

"It's no exaggeration to say that today is a historic day for the Greek economy," said Greek Premier Lucas Papademos, who had rushed to the finance ministers' meeting to lend weight to his country's pleas for help.

The eurozone and the IMF said the deal is expected to bring Greece's debt down to 120.5 percent of gross domestic product by 2020 ? around the maximum that they consider sustainable. At the moment, Greece's debt stands at more than 160 percent of GDP.

The euro spiked 0.5 percent to $1.3264 following the deal but stock markets in Europe were trading slightly lower, having enjoyed solid gains in the run-up to the meeting on the expectation that a deal would be secured.

Even some of the politicians who had pushed hardest for sticking close to the 120 percent target first set out in October conceded that it was questionable whether that level was indeed manageable for such a week economy as Greece.

"One can discuss at length the assumptions on which this (target) is based," German Finance Minister Wolfgang Schaeuble said after the meeting. "Because of that we decided to at least be sincere about the figures."

Ahead of the meeting, Greece's international creditors ? the EU, the ECB and the IMF ? warned that without new measures the debt would still remain close to 129 percent by the end of the decade even under its optimistic scenario. That shortfall persisted even though Athens had faced down violent protests to pass a massive new round of cuts and reforms through Parliament just last week.

So to reach a successful outcome, the finance ministers had to fight on many fronts.

The representatives of private holders of Greek debt had to agree to steeper losses than they had previously said was possible in a voluntary debt relief. The Institute of International Finance said that the bond swap could see Greece's debt reduced by euro107 billion immediately. On top of that, investors will be asked to give Athens 30 years to repay them, compared with just under 7 years. Average interest rates, meanwhile, would fall to 3.65 percent from around 4.8 percent.

Private investors weren't the only ones having to give ground.

The eurozone countries will reduce the interest that Greece has to pay for its first package of bailout loans to 1.5 percentage points over market rates from between 2 percentage points to 3 percentage points currently, cutting both its debt load and limiting the need for new rescue loans.

At the same time, the European Central Bank and the national central banks in the countries that use the euro will forego profits on their Greek debt holdings, again reducing the costs for Greece.

"It's a very good accord in the sense that it is equitably divvied up," said French Finance Minister Francois Baroin. "The Greeks have made their efforts. The Europeans are playing their supporting role, in their role as creditors ... And the private sector part goes beyond" what could be expected.

But several hurdles remain before Greece will see any of the money or other benefits of the new program.

Apart from the implementation of more than 30 different savings and reform measures by Greece, the new bailout has to be debated by parliaments in several member states, including Germany, the Netherlands and Finland.

The IMF also still has to decide how much of the euro130 billion bill it is willing to stump up. Going into the meeting, the Washington-based fund had indicated that its contribution will be lower than the one-third of the total it has provided in previous bailouts.

The IMF's Lagarde said the fund's board would decide on its contribution in the second week of March.

"In doing so it will have in mind the overall program, but also additional matters such as the proper setting up of a decent firewall," Lagarde said with reference to Europe's current and future bailout funds.

At the moment, the overall ceiling for eurozone rescue loans has been set ateuro500 billion ($663 billion), much of which has already been committed to Ireland, Portugal and now Greece. Euro leaders will decide at their summit in early March whether that ceiling should be increased.

On top of that, it will also take some time to see how many private creditors will participate in the debt relief and how many of them will have to be forced to sign up through new legal clauses Greece plans to implement. Some analysts fear that imposing losses on even some bondholders may destabilize markets.

Perhaps most crucially, however, may be new national elections in Greece scheduled for April, which could upend the political landscape in the country. The leaders of the two main parties have committed to the cuts and reform program, but anti-bailout parties have been gaining in the polls.

Strauss-Kahn to be quizzed in prostitution case

Former IMF chief Dominique Strauss-Kahn is to be questioned next week by police investigating an alleged prostitution ring in Lille, northern France, a police source said.

The former finance minister, who left the International Monetary Fund last May to face attempted rape charges in New York that were later dropped, had publicly demanded to be interviewed in the separate Lille case.

Strauss-Kahn has been summoned for 3:00 a.m. EST on Tuesday, the source said, confirming a report in La Voix du Nord newspaper on Saturday. He is to be taken into custody and questioned for up to 48 hours, according to the regional daily.

Messages left for Strauss-Kahn's Paris attorney were not immediately returned.

Once seen as a likely contender against French President Nicolas Sarkozy in upcoming elections, Strauss-Kahn bowed out of public life after his U.S. arrest on allegations that he attempted to rape a Guinean hotel maid.

The maid, Nafissatou Diallo, is now pursuing him in a civil action after prosecutors dropped criminal charges.

Separate attempted-rape accusations by a Parisian writer were also shelved by French police on October 13 - three days before Strauss-Kahn's name surfaced again in the Lille investigation.

The probe is focused on a prostitution ring that allegedly supplied women to clients of the city's luxury Carlton hotel. Local businessmen and a police commissioner are among several people arrested so far.

French construction firm Eiffage dismissed an executive suspected of using company funds to hire prostitutes, who allegedly took part in sex parties with Strauss-Kahn in Paris and Washington.

Strauss-Kahn believed he was participating in swingers' parties and had no reason to suspect that the women were prostitutes, his lawyer Henri Leclerc has previously said. Consorting with prostitutes is legal in France.

(AP) ? President Barack Obama's proposed budget would eliminate the nation's only program that regularly tests fruits and vegetables for deadly pathogens, leaving public health officials without a crucial tool used to investigate deadly foodborne illness outbreaks.

The budget plan the president sent to Congress Monday would ax the Agriculture Department's tiny Microbiological Data Program, which extensively screens high-risk fresh produce throughout the year for bacteria including salmonella, E. coli and listeria.

If samples are positive, they can trigger nationwide recalls, and keep tainted produce from reaching consumers or grocery store shelves.

Food safety advocates and a top-ranking U.S. Centers for Disease Control and Prevention official said the information also can help pinpoint foods tied to illness outbreaks, and would not easily be replaced by companies' internal tests or more modest federal sampling programs.

"It's the radar gun that keeps the industry honest and if that's eliminated, we don't have a program that will keep the industry in check," said Mike Doyle, director of the University of Georgia's Center for Food Safety, which works with the produce industry to improve safety measures on farms and in packinghouses. "This is really important because you and I eat that food and we don't want to get sick."

White House Office of Management and Budget spokeswoman Meg Reilly said the decision to cut the $5 million program was made after USDA decided it had limited impact. She said it also USDA determined it did not fit within its Agricultural Marketing Service division, which is partially funded by fees collected from produce growers.

"While food safety is a vitally important part of successfully marketing produce and other agricultural products, other federal and state public health agencies are better equipped to perform this function," USDA spokeswoman Courtney Rowe said in a statement.

Industry leaders from United Fresh Produce and other major trade associations have pushed the government in recent years to get rid of the comprehensive testing program, saying it has cost growers millions in produce recalls and has unfairly targeted farmers who aren't responsible for contaminating the food. They want the private sector to do more of its own testing, rather than allowing USDA to take random samples of fruits and vegetables at massive grocery store distribution centers, after produce has already left company control.

Last year, for instance, California firms had to recall pre-packaged fresh cilantro and bagged spinach after MDP tests of random samples detected salmonella.

The 120,000 food samples the program has collected in the last decade have offered public health officials important clues when they are probing the source of food poisoning outbreaks, Dr. Robert Tauxe, the CDC's top food-germ investigator, said in an interview in October when the agency began offering the program's employees early retirement packages.

Last year, the program found lettuce and spinach contaminated with E. coli O157:H7, the strain most commonly responsible for food poisoning, and also started aggressively testing for listeria in cantaloupes in response to the nation's most deadly foodborne illness outbreak, in which 30 people died after eating listeria-tainted melons. In one instance in the last several years, a contaminated produce sample the program identified was later tied to an illness cluster, Tauxe said.

FDA Produce Safety Staff Director Samir Assar said in an October interview that while his agency also conducts targeted tests of certain high-risk fresh fruits and vegetables each year, cutting MDP would leave the regular testing of sprouts, tomatoes, cantaloupe and cilantro to industry and more modest state and federal efforts.

"I don't think this would be information that would be easy to replace," Tauxe said.

Rowe suggested the Food and Drug Administration, CDC and state agencies should collect similar data, but no agency reported having immediate plans to step in.

FDA spokeswoman Siobhan DeLancey said she could not speculate on whether FDA would set up a parallel program, or had the funding to do so.

"We don't test produce," said Lola Russell, a CDC spokeswoman. "That's just not part of our mission."

State health departments are already facing tough choices as they try to come up with enough dollars to keep food safe after tens thousands of employees have been laid off in recent years. And the FDA has always been crunched for food safety dollars, receiving so little money for food inspections that some facilities are only inspected every five to 10 years. A new food safety law President Obama signed last year aims to increase the number of inspections in the United States and abroad, but emphasizes prevention rather than increased testing of foods.

Still, both the industry and government agree that tests alone won't keep the food supply safe from contamination.

Since the 2006 E. coli outbreak in spinach that killed three people and sickened more than 200, growers in California's lush Salinas River Valley have been trying out new farming and testing standards for leafy greens, some of which have since become national guidelines.

Nicknamed "America's Salad Bowl," the area grows much of the nation's lettuce, and the local industry collects reams of data about pathogens to improve their growing practices, said Hank Giclas of the trade association Western Growers.

Half a dozen major shippers are currently pooling their data about leafy greens and may share it with other businesses, the government and academic researchers in the future, Giclas said.

"It's possible that private sector testing could serve as a type of substitute. Industry has shown a willingness to share that information, so those are the kinds of things that we need to talk about," Giclas said.
